Summary/Objective: The Direct Support Professional is responsible for providing support to individuals served according to established rules and regulations, including proper documentation of all rendered services.

 

Essential Job Functions:

  • Report issues of concern to the Supervisor. Offer ideas and suggestions for resolution of concerns. Use appropriate methods and channels of communication.
  • Direct Support Responsibilities:
    • Teach, train, and assist with routine personal care of individuals, including bathing, shampooing, hair combing, nail trimming, shaving, brushing of teeth, and dressing. Ensure the neat and clean appearance of individuals served, taking into account the personal choices of the individuals served.
    • Teach, train, and assist individuals with daily living tasks, such as room straightening, general cleaning, and laundry.
    • Assist in the dining room at meal times and monitor individuals.
    • Assist individuals during admittance, including room orientation, checking in of belongings, delivery of personal items to the laundry room for marking, storing belongings, facility orientation, and daily routines and schedules.
    • Assist individuals during discharge, including the collection of personal belongings and the checking off of articles. All documentation must be completed and filed on the day of discharge.
    • Assist with individual program plans and the implementation of program of care, including the development and maintenance of individuals' optimum level of functioning.
    • Prepare the individual for appointments. Take any required paperwork and transport to the appointment, attend the session if the individual chooses and document in the medical chart regarding the appointment.
    • Assist with transporting individuals outside the facility.
    • Offer activities and encourage attendance.
    • Be an active advocate for individuals receiving services to have greater control over their lives. Encourage individual choice and decision-making.
  • Ensure all documentation meets Iowa Code standards utilizing the Edoc system:
    • Complete documentation in a timely manner following service provision. Turn logs in daily.
    • Complete incident/accident and behavior reports accurately and promptly.
    • Complete service documentation to record interventions and supports provided and reflect the goals and ongoing supports of the individuals served.
    • Complete service documentation to record individual responses to interventions and supports.
  • Safety Responsibilities:
    • Provide for the safety of the individuals served with the least amount of intrusion as possible.
    • Respect the property of individuals served (e.g., personal possessions, etc.).
    • Complete safety checks according to facility procedures.
    • Immediately report any unusual observations, occurrences, or behavior changes to thenursing staff and document these incidents.
    • Follow established protocols to de-escalate situations involving individuals' verbal and physical aggression by utilizing CPI techniques.
  • Scheduled work shifts:
    • Find a replacement if unable to work a scheduled shift.
    • Follow the department call-in procedure.
    • Work weekends and holidays as scheduled.
  • Other job duties as assigned.

  • Report to work on time and as scheduled. Regular and predictable attendance is an essential element of this position. Patterns of absences or tardiness are not acceptable.

  • Portray a positive image of Imagine the Possibilities, Inc. when representing the agency.
  • Build a sense of unity and purpose among coworkers, supervisors, and individuals served by working as a team member.
  • Ensure all Imagine the Possibilities, Inc. policies and procedures are followed, including the Compliance and Integrity Program and Safety.

Work Environment: This job operates in a licensed facility setting.

Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

    • May require lifting at times, up to 35 pounds. Lifting more than 35 pounds requires the use of a two-person lift, mechanical lift, or assistive equipment.
    • May require prolonged sitting and working at a computer for up to eight hours.
    • May require frequent bending, stooping, and stretching.
    • May require prolonged standing for up to eight hours.
    • Must wear hearing and eye protection in required areas.
    • Must have ability to see objects/persons at a distance.
    • Must have ability to see close work such as typed or handwritten material.
    • Must have ability to hear conversations in a quiet and a noisy environment.
    • Must have ability to determine where a sound is coming from.
    • Must have ability to hear differences among bells, buzzers, beeps, horns, etc.
    • Must have ability to communicate through speech.

Travel: Limited travel is expected for this position.

Preferred Education and Experience:

    • At least 18 years of age with High School Diploma or GED.
    • Experience working with people with disabilities preferred.
    • Basic computer skills including MS Word.
    • Background checks with previous employers, references, law enforcement agencies, and others will be conducted and found satisfactory.
    • Valid driver's license and driver's record deemed acceptable by Imagine the Possibilities, Inc.
